Eye Check Procedure
Statement on the Eye Check (Restricted Instruction Permit) process by the Commissioner of the DMV
During the 30 hour classroom phase of Driver Education, a representative from the School Bus and Traffic Safety Division of the DMV will come to our classrooms to conduct a vision and medical screening (commonly referred to as an "eye check") of all students. The only exception to this procedure is for students attending our Saturday classes (see Saturday class eye check procedures here). Students who successfully complete this screening are issued a Restricted Instruction Permit which allows a student to drive a Driver Education vehicle during their behind-the-wheel training with our certified Driver Education teachers.
Without a completed Restricted Instruction Permit that has been signed by a DMV representative, students cannot be scheduled for their behind-the-wheel classes. Students who are absent on the day of the vision exam, students who forget their corrective lenses on the day of the vision exam, students whose paperwork was not filled out correctly or who did not turn their Restricted Instruction Permit back in to the teacher with a parent signature before the day of the vision exam will not have a Restricted Instruction Permit that has been signed by a DMV representative.
Also, those students who are given the vision screening during the 30 hour classroom phase, but failed the vision screening, will not have a Restricted Instruction Permit signed by a DMV representative. Students in need of seeing a vision specialist will be issued an 804 form by the DMV representative that comes to our classes. See here for more information on 804 forms and procedures.

If any of the above reasons kept the student from obtaining the DMV specialist’s signature, then a letter will be sent to the student’s home address with instructions on how to rectify the situation. Usually, this letter is mailed out approximately 2 weeks after the student’s class has ended.
Students will not be put into the "ready to drive" file until the student has taken the vision screening, and we have a completed Restricted Instruction Permit on file in the Jordan Driving School office.
